138% is the Medicaid line in states that adopted the Affordable Care Act expansion. The statute itself names a lower figure and then disregards five percentage points of income on top of it; the two added together are what every state screener and every article prints. That is why 138% is everywhere and appears in no table HHS publishes.
Whether the line applies to you at all depends on your state: those that did not adopt the expansion set their own adult limits, which are far lower and are not multiples of this guideline. Medicaid also tests monthly income for most groups, so these annual figures have to be divided down under the state’s own rule before they mean anything.
